Registration fee for 2017 is N$350.00 per application.

Forms are printed in A3 format and cannot be e-mailed, we can however send you the document or you are welcome to collect:

Requirements:

Yearly membership fees: as prescribed by Department of Works annually

Please complete form in full and have Commissioner of Oaths stamp form.
We require that you give us the following documentation:

1. Letter confirming employment at registered Namibian Practice

2. Copy of Namibian Identity document or passport

3. Foreigners: copy of work permit

4. Copy of qualification
